Only our Health History & Registration form will need to be completed prior to your child's first visit. You will not need to print the additional forms or bring them with you to your appointment, as we will have you sign electronically for them once you arrive for your initial appointment at our office.
Please complete our New Patient Health History and Registration Form by clicking the "fill out online" link below, before your child's dental appointment. If you have more than one child being seen for a new patient visit, please complete one submission for each child. If you are unable to complete the forms online prior to arriving for your child's first dental visit, we ask that you arrive 30 minutes in advance to complete these forms and avoid having to reschedule your child's appointment.
A printed copy of this form will be signed in the office at your first visit.
